Out-of-Network Benefits
As a courtesy, we can direct bill insurance companies for patients who have out of network benefits. Please contact your insurance company to verify your out of network benefits.

Medicare
We are a NON-PARTICIPATING PHYSICIAN with Medicare and have opted-out of the Medicare program. We do not file Medicare claims. Medicare Part B beneficiaries will be required to sign a standard affidavit pursuant to section 4507 of the Balanced Budget Act of 1997 and will not be permitted to submit receipts for reimbursement. We will only file a claim with Medicare for emergency or urgent care.
